29 CFR 1910.333(b)(2)(ii)(A): Safe procedures for deenergizing circuits and equipment were not determined before circuits or equipment are deenergized. On or about August 14, 2023, safe procedures for deenergizing all electrical energy sources located in the outdoor tank farm metal raceway were not determined prior to employees working to replace/reinstall the conductors. The Active Group Lockout #30 "Tank Farm" procedures did not identify and address the 'Truck Loadout Pump' circuit, energized through circuit breaker #17 in the MSP-3 panel located in the DDE MCC room.

29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that the circuit elements and equipment parts were deenergized: On or about August 14, 2023, a qualified person did not use test equipment to test and verify a 480-volt conductor for the 'Truck Loadout Pump' circuit, energized through circuit breaker #17 in the MSP-3 panel located in the DDE MCC room was deenergized prior to work started to replace wiring in the outdoor raceway.
