29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace: a) On or about 09/06/2023, in the warehouse; employees were exposed to struck by hazards when the employer did not ensure a formal Powered Industrial Vehicle (PIV) Training Program was in place and provided to new employees operating PIVs. Employees operated PIVs such as, but not limited to: Caterpillar forklifts to move pallets of materials throughout the building. NO ABATEMENT DOCUMENTATION REQUIRED

Employees use PIVs such as, but not limited to: Caterpillar forklift trucks, for the purpose of moving materials throughout the facility. NO ABATEMENT DOCUMENTATION REQUIRED
