29 CFR 1910.261(b)(1): Lockouts. Devices such as padlocks shall be provided for locking out the source of power at the main disconnect switch. Before any maintenance, inspection, cleaning, adjusting, or servicing of equipment (electrical, mechanical, or other) that requires entrance into or close contact with the machinery or equipment, the main power disconnect switch or valve, or both, controlling its source of power or flow of material, shall be locked out or blocked off with padlock, blank flange, or similar device. On or about August 30, 2023, at an establishment located at 600 Thilmany Road, in Kaukauna, Wisconsin; at Paper Machine 14, the main power disconnect switch and valves were not locked out prior to employees removing slabs hung up on the trim chute on PM14 pulper exposing employees to caught in hazards while the pulper was still energized and operating.
