29 CFR 1926.451(e)(1): Safe access was not provided to scaffold platforms when the platforms were more than 2 feet above or below the point of access:   a) On September 1, 2023 at 3920 N Kostner Avenue, Chicago, IL, employees were exposed to fall hazards while accessing and egressing a ladder scaffold with a work platform 19 feet above the ground.  In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:  a) On September 1, 2023, at 3920 N. Kostner Avenue, Chicago, IL, employees conducting siding work were exposed to fall hazards while working on a ladder jack scaffold 19 feet above the ground without fall protection.  In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of equipment, photographic or video evidence of abatement or other written records.
