Philadelphia, PA - On or about August 23, 2023, the employer did not ensure that employees performing roofing operations from the upper and lower roofs of a single story commercial building were protected from fall hazards by the use of a fall protection system. a) From the upper level roof, approximately 6 feet and 8 inches above the lower level roof. b) From the lower level roof, approximately 15 feet above the ground level.

29 CFR 1926.503(a)(1): Employer did not provide fall protection training to those employees who might be exposed to a fall hazard. Jobsite: 3200 Red Lion Rd. Philadelphia, PA - On or about August 23, 2023, the employer did not provide effective training for each employee conducting roofing operations from the upper and lower level roofs of a commercial building, and exposed to fall hazards between approximately 6 feet and 8 inches to 15 feet in the recognition and avoidance of fall hazards.
