29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):   a)  Production Floor, Capper Conveyor/A004 - On or about 8/25/23, the employer did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance, due to the employer's lack of development of Lockout/Tagout procedures for the piece of equipment in the A-line where employees were performing cleaning and maintenance of the conveyor.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:   a)  Throughout Establishment - On or about 8/25/23, the employer did not provide adequate lockout/tagout (LOTO) training to ensure authorized employees were able to recognize applicable hazardous energy sources and the methods and means necessary for energy isolation to protect themselves and others from hazardous energy and amputation injuries while cleaning equipment, including, but not limited to, the Capper conveyor.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Production Floor, A-Line, Capper/A004 - On or about 8/25/23, the conveyor and sprocket in-running nip point was not adequately guarded in that the metal door enclosures were not provided with a mechanical/electrical interlock, nor was the nip point itself provided with an adequate guard.  Denison Pharmaceuticals, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1625664, citation number 1, item number 1, and was affirmed as a final order on 2/10/2023, with respect to a workplace located at 1 Powder Hill Road, Lincoln, RI 02865.  Denison Pharmaceuticals, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1617550, citation number 1, item number 1, and was affirmed as a final order on 2/10/2023, with respect to a workplace located at 1 Powder Hill Road, Lincoln, RI 02865.
