29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  The employer was failing to protect employees from electrical shock hazards. This was most recently documented at a worksite, at or near, 1021 North 3rd St., O'Neill Nebraska on or about September 7, 2023. An employee(s) was using an electrical box attached to the end of a flexible cord, not attached to a wall.  The electrical box and cord were being used to power a Milwaukee Sawzall.

29 CFR 1926.404(b)(1)(iii)(C): A cord set and plug was not visually inspected before each day's use for external defects, such as insulation damage and such equipment was defective and used:  The employer was failing to protect employees from electrical shock hazards. This was most recently documented at a worksite located at or near, 1021 North 3rd St., O'Neill, Nebraska on or about September 7, 2023. An employee was using a flexible cord to power a Milwaukee Sawzall that had damaged insulation.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented:   Employees engaged in activities at the residential construction site located at 1021 North 3rd St., O'Neill, Nebraska, were exposed to fall hazards in that employee(s).  This exposure was documented on or about September 7, 2023.  Employee(s) were exposed to a fall hazards in the following but not limited to:  a)	Employee(s) were working on the roof without any form of fall protection.  The height of the roof was measured at 10 feet from the eave to the ground.   b)	Employee(s) were working a a height greater than 10 feet from the bucket of a John Deere Skidsteer without any form of fall protection.   c)	Employee(s) were exposed to a fall greater than 6 feet when working near an open staircase.    d)	Employee(s) engaged in building wood trusses in the attic of the structure at a height of approximately 9 feet above the next lower level were not using fall protection 100% of the time as required by this sub-part.
