29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:   Location: 26B Commerce Rd., Fairfield, NJ 07004  a:  The employer did not ensure that each powered industrial truck operator had successfully completed training consisting of a combination of formal instruction and practical training.  Employees assigned to operate Crown Order Picker, SP 3400 series did not receive formal instruction and/or evaluation prior to operating the equipment.   Violation was observed on 09/11/2023
