At the jobsite, located at Cox Creek Parkway, Florence, AL: On or about 9/11/2023, the employer exposed employees to struck-by hazards, in that protective helmets were not provided to employees while they were working underneath employees replacing roof shingles.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  a) At the jobsite, located at Cox Creek Parkway, Florence, AL: On or about 9/11/2023, the employer exposed employees to struck by hazards, in that eye protection was not used while conducting roofing operations with pneumatic nail guns. 29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) At the jobsite, located at  Cox Creek Parkway, Florence, AL: On or about 9/11/2023;  the employer exposed employees to fall hazards, in that fall protection was not utilized by employees while performing roofing work.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:.  a) At the main entrance, on the south side of the building , located at 610 Cox Creek Parkway, Florence, AL: On or about 9/11/2023, the employer exposed employees to fall hazards, in that employees replacing shingles on a commercial structure using a portable ladder to access the landing surface did not extend 3 feet above the landing surface.   b) At the side entrance, on the east side of the building , located at 610Cox Creek Parkway, Florence, AL: On or about 9/11/2023;  employees replacing shingles on a commercial structure using a portable ladder to access the landing surface did not extend 3 feet above the landing surface.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
