29 CFR  1926.251(a)(2)(i): Employer did not ensure that rigging equipment had permanently affixed and legible identification markings as prescribed by the manufacturer that indicate the recommended safe working load;  Jobsite - The employer used a self designed and installed steel sleeve attached to a all-terrain fork lift for lifting and setting trusses without prior manufacturer approval and load rating.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Facility - On or about September 11, 2023, employees were working on studded wall headers, which were approximately 16 feet from the ground, to place trusses without use of personal fall arrest systems.

29 CFR  1926.1053(b)(20):Employees did not face the ladder when ascending or descending the ladder:  Facility - An employee using a non self-supported ladder to travel down from the wall header to the ground, was facing away from the ladder while climbing down.
