The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. The employer was failing to protect employee(s) from fall injuries. This was most recently documented on September 11, 2023, at 13255 West Center Road Omaha, Nebraska. The employer failed to ensure that employee(s) were not exposed to fall hazards while operating and working from an aerial lift by utilizing a safety harness with attached lanyard when in the basket of the aerial lift.
