29 CFR 1910.145(f)(3):Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent: a) Facility: On or about September 12, 2023, and times prior to, employees were exposed to struck by hazards from a broken overhead commercial garage door that was not taken out of service or repaired according to manufactures specifications. The door was held in an open position by a single piece of rebar and was tied back at the top of the door and secured with C-clamps.
