29 CFR  1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Location: Worksite  On or about September 12, 2023, employees were exposed to fall hazards of over 20 feet while walking and working on roof trusses and wearing full body harnesses without lanyards, and not secured. Frequent and regular inspections of the jobsite by a competent person were not performed to ensure that employees were tied off properly.
