29 CFR 1910.303(g)(1)(i): The dimension of the working space in the direction of access to live parts operating at 600 volts or less and likely to require examination, adjustment, servicing, or maintenance while alive was less than indicated in Table S1:  1146 Sheridan St, Chicopee, MA:  On September 12, 2023, and at times prior, the employer did not ensure sufficient access and working space was provided for the 480 volt Square D 3 phase disconnect for Rotary Die Cutter # 44, to permit ready and safe operation and maintenance.
