29 CFR  1910.28(b)(3)(iv):  The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  BlueScope Recycling and Materials, LLC - Mansfield, Ohio: On or about September 7, 2023, the employer did not ensure that the ladderway platform hole, located on the elevated platform to access the Transfer Line 4 waste conveyor, was guarded to protect employees from fall hazards of approximately 17 feet.  No additional abatement information is needed for this item.

29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  BlueScope Recycling and Materials, LLC - Mansfield, Ohio: On or about September 7, 2023, the employer did not develop, document, and use energy control procedures for the Transfer Line 4 waste conveyor located at the Insulated Copper Wire (ICW) building. Employees were exposed to injuries associated with being caught-in rotating parts while maintaining the equipment.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a.  BlueScope Recycling and Materials, LLC - Mansfield, Ohio: On or about September 7, 2023, an employee performing cleaning activities at the Transfer Line 4 waste conveyor, located outside the Insulated Copper Wire (ICW) building, sustained an arm amputation injury after being caught in the equipment's rotating tail pulley.  The employer did not ensure that energy control application steps were implemented to control the equipment's electrical/mechanical energy source prior to the maintenance task - the employer did not: - Ensure employees had the method or means to control hazardous energy. - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished.  b.  BlueScope Recycling and Materials, LLC - Mansfield, Ohio:  On or about September 7, 2023, employees who performed cleaning activities at the Transfer Line 4 waste conveyor, located outside the Insulated Copper Wire (ICW) building, were exposed to injuries associated with being caught-in a rotating part and nip point created by the equipment's tail pulley and conveyor belt. The employer did not ensure that energy control application steps were implemented to control the equipment's electrical/mechanical energy source prior to the maintenance task - the employer did not: - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished.
