29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a)  On 09/12/2023, in the Range Control Welding Shop, the gap between the tongue and the abrasive wheel on one side of the Jet bench grinder exceeded one fourth of an inch. The other wheel was used without the tongue guard.  This employed employees to amputation hazards.  (b)  On 09/12/2023, in the Golf Course Grounds Equipment Shop the adjustable tongue of the Dayton bench grinder was not used. This employed employees to amputation hazards.

29 CFR  1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:   (a)  On 09/12/2023, in the materials yard at Range Control, the employer did not ensure that employees were protected from fall hazards while handling materials in the yard. Employees used a facility-made forklift man basket as a raised work platform and were exposed to fall hazards exceeding 4 feet above a concrete floor due to a lack of guard rail on one side.  The basket was not attached nor secured to the forklift. Employees rode in the basket while a supervisor operated the forklift. Employees were not wearing fall arrest or positioning devices. This condition exposed employees to fall hazards and forklift tip-over hazards.
