29 CFR 1926.502(k)(10): The employer did not reevaluate the effectiveness of written fall protection method planning after accidents, near misses, or observed employee deficiencies: On or about September 13, 2023, and at times prior thereto, workers installing floor joists and A-frame trusses were exposed to fall hazards due to the employer not assessing deficiencies of the fall protection plan with regard to the hazard of the instability of A-frames or joists and the requirement not to use them as supports.
