29 CFR  1910.28(b)(13)(ii): The employer did not ensure that when work is performed at least 6 feet (1.6 m) but less than 15 feet (4.6 m) from the roof edge, each employee is protected from falling by using a guardrail system, safety net system, travel restraint system, or personal fall arrest system. The employer may use a designated area when performing work that is both infrequent and temporary.  a) Budget Window Cleaning LLC. On or about September 12, 2023, at the site location of 31941 Walker Rd. Avon Lake, OH: The employer did not ensure employees engaged in cleaning activities upon a 4/12 low slope roof were adequately protected from falls.  An employee engaged in window cleaning in this condition was exposed to a fall of 11 feet to the ground below.

29 CFR 1910.30(a)(3): The employer did not train each employee in at least the following topics listed in 1910.30(a)(3)(i)-(iv):  a) Budget Window Cleaning LLC. On or about September 12, 2023, at the site location of 31941 Walker Rd. Avon Lake, OH: The employer failed to effectively train employees in the recognition and avoidance of hazards associated with working from heights. Employee engaged in window cleaning activities atop a low slope residential structure without such training were exposed to a 11 feet fall to the ground below.
