29 CFR  1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a)	On or about August 14, 2023, and times prior thereto, where employees perform maintenance on electrical equipment without the use of an energy control program.

29 CFR  1910.303(f)(2):  Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, were not legibly marked to indicate its purpose, unless located and arranged so the purpose is evident:  On or about August 14, 2023, and times prior thereto, in the electrical closet for the air conditioning system, where employees were exposed to electrical hazards while locating feeder services that were not legibly marked.

29 CFR  1910.333(b)(2)(iv)(B):  A qualified person was not used to test equipment to test the circuit elements and electrical parts of equipment to which employees will be exposed and did not verify that the circuit elements and equipment parts are deenergized. The test did not determine if any energized condition exists as a result of inadvertently induced voltage or unrelated voltage backfeed even though specific parts of the circuit have been deenergized and presumed to be safe:  On or about August 14, 2023, where  unqualified employees were exposed to electrical hazards while tracing a 140 volt electrical wire to the breaker box to be shut off.
