On or about September 14, 2023, in various locations, employees were operating machines without proper or adequate guarding: a) An employee, an edger operator, was exposed in the power roll area on the Northwest side of the mill.  b) An employee was working near an exposed pinch point in the tie dock area on the south side of the mill.

29 CFR 1910.334(a)(2)(i): Portable cord and plug-connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to the outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  On or about September 14, 2023, employees on the Northeast side of the mill were using a pedestal fan, and the power cord was damaged. The outer insulation was pulled, exposing the inner wiring. Employees were engaged in daily operations and were exposed to electrical shock hazards due to the employer's failure to inspect the flexible cord used to power fans visually. The pedestal fan was in full operation when inspected.
