On or about September 7, 2023 a trainee was struck by a Raymond electric pallet jack operated by another trainee during the course of a practical skills assessment.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization. On or about September 7, 2023 an employee was injured and transported to the hospital by ambulance. The employer confirmed the employee was still in the hospital approximately 24 hours post incident, received confirmation of inpatient status six days post incident, and reported to OSHA seven days post incident.
