29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:   a) On or about 9/15/2023, at the location of a new Dollar General Store Commercial Site on 330 Center Rd. West Seneca, NY;  Employee who was standing on the wood platforms attached to the forks of a Skytrak lift truck, was not protected from falling. Employee was exposed to fall of 15 feet.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:   a) On or about 9/15/2023, at the site in West Seneca NY. Employee was on the wood platforms attached to the forks of a Skytrak lift truck, exposing employee to a fall of 15  feet. Modification of lift truck was made without the manufacturer's written approval.   NO ABATEMENT CERTIFICATION REQUIRED
