29 CFR 1928.57(a)(6)(iii): Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment: On or about September 11, 2023, and at times thereto, at La Feria Co-Op Gin & Supply, employees engaged in cotton gin activities were not instructed on the procedures and steps necessary to ensure all systems were turned off and locked out prior to dislodging a cotton bale from the Lummus press ram, exposing employees to caught-in and crushing hazards.
