29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) CNC Room - On or about September 13, 2023, at the above location, the employer failed to provide effective guarding on the "Bur-Beaver" Deburring and Beveling Machine, Serial #92852, thereby, exposing employees to laceration, fracture and amputation hazards from rotating parts.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) CNC Room - On or about September 13, 2023, at the above location, the employer failed to ensure that the "Bur-Beaver" Deburring and Beveling Machine, Serial #92852, was equipped with a ground pin on its electrical plug, thereby exposing employees to an electrocution hazard.
