29 CFR  1926.102(a)(1):Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  At a residential worksite located at 8868 N Poplar Ave, KC MO -- employee was engaged in roofing duties were using a pneumatic nail gun without eye protection. This exposed the employee to getting debris in his eyes. This was documented on or about 09/19/2023.  Osvaldo Prieto-Garcia was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1608011, citation number 1, item number 1, and was affirmed as a final order on 02/15/2023, with respect to a workplace located at 18000 E 18th St N, Independence, MO 64058.

This exposed employees to fall hazards of 11 feet above ground level. This was documented on or about 09/19/2023.  Osvaldo Prieto-Garcia was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1608011, citation number 1, item number 2, and was affirmed as a final order on 02/15/2023, with respect to a workplace located at 18000 E 18th St N, Independence, MO 64058.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a residential worksite located at 8868 N Poplar Ave, KC MO -- employee was engaged in roofing duties and using an extension ladder to gain access to the roof. The ladder was not 3 rungs (feet) above the landing surface. This exposed the employee to fall hazards 11 feet to ground level. This was documented on or about 09/19/2023.
