29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support: On or about September 20, 2023, employees were exposed to fall hazards of approximately 10 feet while accessing and exiting the roof via an extension ladder which, due to the height of the ladder, did not extend 3 feet above the roof being accessed and the employer had not ensured the ladder was secured to a rigid support.
