29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  (a) On or about Sept 20, 2023, at 8419 Corliss Ave. Sturtevant, WI 53177, frequent and regular job site inspections were not conducted to identify and correct safety hazards, including but not limited to fall hazards and struck by hazards.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) On or about Sept 20, 2023, at 8419 Corliss Ave. Sturtevant, WI 53177, the employer did not ensure that an employee working on a pump jack scaffolding at an approximate height of 13 feet 7 inches was protected from    falls to the lower level with fall protection.

29 CFR  1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  (a) On or about Sept 20, 2023, at 8419 Corliss Ave. Sturtevant, WI 53177, the employer did not ensure that all employees that work on pump jack scaffolding were trained to recognize hazards associated with working on scaffolding thus exposing employees to electrical hazards, fall hazards and falling object hazards.  (b) On or about Sept 20, 2023, at 8419 Corliss Ave. Sturtevant, WI 53177, the employer did not ensure that all employees that work on the Genie S-60X telescoping boom lift  were trained to recognize hazards associated with working on scaffolding thus exposing employees to electrical hazards, fall hazards and falling object hazards.
