In production  lines 1, 2, 3 and 4, where production of grain mixture and packaging takes place, employee were exposed to a heavy fugitive grain dust accumulation and the excess spillage of pellet product on the work floor. The employer had not developed and implemented a written housekeeping program. This was observed on 9/20/2023 during first shift work.

Employees engaged in production activities in Line 3 in the shop were exposed to fire and electrical shock hazards in that the flexible cord used to supply power to a large industrial fan next to Line 3 was used in lieu of fixed wiring. Flexible cord was also used to supply power to the computer function of production line 3.
