29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. In production lines 1, 2, 3 and 4, where production of grain mixture and packaging takes place, employee were exposed to a heavy fugitive grain dust accumulation and the excess spillage of pellet product on the work floor. The employer had not developed and implemented a written housekeeping program. This was observed on 9/20/2023 during first shift work.

29 CFR 1910.303(g)(2)(i):Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): Employees engaged in production activities at the Cloud Robert's Packaging Equipment, Serial No. 3940 machine in the shop were exposed to fire and electrical shock hazards in that the electrical lines used to power the equipment, were not guarded in an approved enclosure.

29 CFR 1910.305(g)(1)(iv)(A):Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: Employees were exposed to fire and electrical shock hazards. Employees engaged in production activities in Line 3 in the shop were exposed to fire and electrical shock hazards in that the flexible cord used to supply power to a large industrial fan next to Line 3 was used in lieu of fixed wiring. Flexible cord was also used to supply power to the computer function of production line 3.
