29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) USPS Memphis MPA, warehouse floor: On or about 8/11/2023, and at times prior, the employer exposed employees to struck by and other PIT related hazards, in that the Manager of Distribution Operations (MDO) for tour 1 was operating forklifts, Tuggers, and Pallet Jacks without being evaluated since 5/29/2017. (b) USPS Memphis MPA, warehouse floor: On or about 8/11/2023, and at times prior, the employer exposed employees to struck by and other PIT related hazards, in that the Supervisor of Distribution Operations (SDO) for tour 3 was operating forklifts, Tuggers, and Pallet jacks without being evaluated since 2018. (c) USPS Memphis MPA, warehouse floor: On or about 8/11/2023, and at times prior, the employer exposed employees to struck by and other PIT related hazards, in that the Acting Supervisor (204B) was operating forklifts, Tuggers, and Pallet jacks without being evaluated since 8/5/2020.
