Harbor Rd. Lakeside-Marblehead OH 43440, did not ensure that employee(s) operating an aerial lift donned and secured their personal fall arrest systems before operating a JLG 600S aerial lift (Serial Number 0300178777). Employees performing steel erection work activities including siding and roofing on a commercial storage structure were exposed to falls of 25ft or greater to the ground below.
