29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):   On or about September 21, 2023, at Peters 1-6, an employee was not in use of fall protection at the service rig's operators platform which lacked guardrails. This exposed employees to fall hazards greater than 4 feet to a lower level.

29 CFR  1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  On or about September 21, 2023, at Peters 1-6, training on personal fall protection systems was not provided to employees who were required to wear a personal fall protection harness for work at the derrick board which exposes employees to fall hazards.

29 CFR  1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components are removed from service.   a. On or about September 21, 2023, at Peters 1-6, a personal fall protection harness  had a rust at a metal latch, frayed fabric edging, and was used by an employee to climb the derrick ladder which exposed the employee to fall hazards.   b. On or about September 21, 2023, at Peters 1-6, ladder climb assist hooks did not require 2 separate consecutive movements to open the hooks, and a hook had a broken latch.  The climb assist hooks were used by an employee to attach to a personal fall protection harness to climb the derrick ladder which exposed the employee to fall hazards.

29 CFR  1910.140(c)(13)(i):  Anchorages except window cleaners' belt anchors covered by paragraph (e) of this section, were not capable of supporting at least 5,000 pounds (22.2 kN) for each employee attached; or (c)(3)(ii) were not designed, installed, and used under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two.  On or about September 21, 2023, at Peters 1-6, an employee climbed the service rig's fixed ladder and did have an anchorage attached to their personal fall protection capable of supporting at least 5,000 pounds.  The employee was exposed to falls from the ladder greater than 24 feet to a lower level.
