29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and lifesaving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE. On the Cement Barge: Employees were exposed to an approximate 15-foot fall while breaking concrete without wearing a personnel fall arrest system. Employees were exposed to fall hazards.
