COMPANY-WIDE: On or about and at times prior to September 25, 2023, the employer did not accurately complete OSHA Form 300 in that: a) On or about September 14, 2023, an employee sustained an injury to to his right upper arm. The injury would be appropriately captured with a check in Column H (Days Away), the number of days away in Column K, and a check in Column M1.
