29 CFR  1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.   Jobsite:  The extension cord, used by employees to power an electric air compressor, was not protected by a GFCI.

The electric outlet, used by employees to power the air compressor via an extension cord, was not equipped with a ground. 2. The grounding pin of the electric cord, used to power the compressor, was broken off.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Jobsite:  The siderails of the extension ladders, used by workers to access the roofs of the home and detached garage, did not extend 3-ft beyond the roof surface accessed.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Jobsite roof 40 Pine Ridge Rd. Newton (Waban), MA:  The employer did not ensure that employees, engaged in roofing work on the roof of the home and detached garage, were protected from fall hazards greater than 13-ft from open sides and edges by guardrails, safety nets or personal fall arrest systems.  The employer routinely fails to assure effective protection against fall hazards as required by the standard, as demonstrated on or about September 21, 2023, when employees were not protected from fall hazards where fall protection was not utilized at the site at 40 Pine Ridge Rd. Newton (Waban), MA, 02468.  First Nationwide Construction Corp. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1656918, citation number 1, item number 2 and was affirmed as a final order on April 25, 2023, with respect to a workplace located at 191 Blue Hills Parkway, Milton, MA, 02186.
