The employer did not develop, document and ensure employees utilized written hazardous energy control procedures for equipment including, but not limited to the stacker.

29 CFR 1910.265(c)(4)(iv): Elevated walk(s) and runway(s), 4 feet or more from floor level, were not provided with a standard railing:  On or about September 26, 2023, at the Stacker, employees worked from the operator's platform located greater than 4-feet above the lower level without a standard railing installed in that: a.  West side top rail was measured at approximately 33 ? inches. b.  East side top rail was measured at approximately 33 inches. c. West side railing did not have midrails or equivalent intermediate members installed. d. East side railing did not have midrails or equivalent intermediate members installed.  On or about September 26, 2023, at the Stacker, employees worked from the stick layer platform located greater than 4-feet above the lower level without standard railing installed in that: e. West side top rail was measured at approximately 36 1/4 inches. f. East side top rail was measured at approximately 36 1/4 inches. g. West side railing did not have midrails or equivalent intermediate members installed. h. East side railing did not have midrails or equivalent intermediate members installed.
