Worksite, 222 Hudson Street, Copiague, NY 11726- The employees were exposed to caught between, crushing, and struck-by hazards while working around a Beeline Model AA Heavy Duty Alignment Runway System (Serial#: 3698) with a single piston pneumatic lift that did not have a safety pin installed and without a secondary Load-Holding means; on or about 9/26/2023 Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.135(a)(1):The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a) Worksite, 222 Hudson Street, Copiague, NY 11726- Employees were exposed to the hazard of being struck-by the elevated bus while replacing bearings in the rear differential pinion of a 2011 Chevy Bus and were not wearing head protection, on or about September 26, 2023. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
