29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:  6342 Diego Lane, San Antonio, TX 78253: On or about September 26, 2023, and times prior thereto, employees were exposed to a fall hazard of greater than 6 feet, while working on the roof of a 2-story residential home, without being trained on fall hazards recognition.
