Section 5(a)(l) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, including severe burns, to employees in that employees were exposed to fire, flash fire, deflagration, or explosion hazards from an improperly maintained dust collection system and duct work for combustible wood materials located inside a building :  (a) On or about September 27, 2023:  1) In the 990 and 994, and AKRO CNC Pre-hanging Door, Machine Areas of the Production Area:       (a) The dust collection system's nonmetallic dust collection duct work was not properly grounded.       (b) The dust collection systems flexible duct work was longer than necessary to connect equipment to the dust collection system.      (c) Metal fragments including screws were allowed to enter the dust collection system with no means of separating the metal from              the dust collectors, and no fire or explosion mitigation provisions.  2) In the Woodworking Areas, along the south wall, and in the northeast corner of the Production Area, dust collection systems are located within 20 feet of emergency egress routes and exits.  3) The accumulated wood dust was not removed daily from the dust collector's accumulation bags.
