29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m), or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about September 27, 2023, one employee was installing a new roof on a maintenance shed at a residential apartment complex up to approximately 10ft in height without the use of fall protection, exposing the employee to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. On or about September 27, 2023, one employee was using a ladder to access a roof up to approximately 10 feet high without the ladder being extended at least 3 feet above the upper landing surface, exposing the employee to a fall hazard.
