At the jobsite, located at 2631 Midsweet Ave Saint Cloud, FL 34772 ; On or about 9/25/23; the employer exposed employees to electrical shock hazards, in that, a Pentair Superflo High Performance Water pump containing a damaged power cord, with exposed conductors, and was utilized to remove excess water from an excavation.

At the jobsite, located at 2631 Midsweet Ave Saint Cloud, FL 34772 ; On or about 9/25/23; the employer exposed employees to entrapment and crushing hazards, in that, precautions necessary to protect employees adequately from cave-ins was not provided while employees conducted excavation of class-c soil for a residential home pool to be installed.

At the jobsite, located at 2631 Midsweet Ave Saint Cloud, FL 34772 ; On or about 9/25/23; the employer exposed employees to entrapment and crushing hazards, in that, competent monitoring to protect employees from a potential cave-in was not provided as an employee stood within an excavation filled with mud and water mixture, approximately 6 inches in depth.
