29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: (a) Behind Shop Building - On or about September 27, 2023, an employee was exposed to a fire hazard while replacing the integrated fuel pump of a propane fuel system because they were permitted to service the system without first verifying that the fuel tank was depressurized.
