29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about August 31, 2023 at 950 Lake Road, Medina, OH 44256, the employer failed to ensure employees adding oil to the oiler were trained as authorized employees in that the pneumatic oiler has stored pneumatic energy which caused a serious bodily injury.
