On or about September 19, 2023, at 2521 Harn Boulevard, Clearwater, Florida, 33764, employees were engaged in roofing work; wherein the employees were removing roof shingles and installing self-adhering base sheets/vapor barrier on a 2-story steep-slop roof with unprotected sides and edges 6 feet or more above lower levels, during which time the employees were not protected from falling by either a guardrail systems, safety net systems, personal fall arrest systems, therefore exposing the employees to a fall hazard.  b. On or about September 19, 2023, at 2521 Harn Boulevard, Clearwater, Florida, 33764, an employee was engaged in roofing work; wherein the employee was throwing damaged/unusable rolls of self-adhering base sheets/vapor barrier from the roof and onto the ground from a 2-story steep-slop roof with unprotected sides and edges 6 feet or more above lower levels, during which time the employees were not protected from falling by either a guardrail systems, safety net systems, personal fall arrest systems, therefore exposing the employees to a fall hazard.
