29 CFR  1926.451(f)(3):  Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  On or about September 28, 2023, an employee was using a scaffold with fence posts used as guardrails and the scaffold components were not inspected for visible defects. This exposed the employee to a safety hazard.

29 CFR  1926.451(g)(4)(xi):  Guardrails were not surfaced to prevent injury to an employee from punctures or lacerations, and to prevent snagging of clothing.  On or about September 28, 2023, employees used a scaffold that had guardrails made of t-posts, with sharp metal protrusions. This exposed employees to safety hazards.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:  On or about September 28, 2023, an employee was working from a canopy of a store without the use of fall protection. This exposed the employee to fall hazard greater than six (6) feet to the ground below.
