29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) On or before October 02, 2023, and at times prior, the employer did not ensure that employees were protected from fall hazards in excess of 20 feet while conducting roofing activities. This condition exposed worker to fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or before October 02, 2023, and at times prior the employer did not ensure that employees had improperly installed two portable extension ladders that were used to access the roofs of the residential home. Ladders being utilized to access each residential roof were not installed to extended at least 3 feet above each landing surface and were not secured to the structure. This condition exposed subcontractor employees to a fall hazard in excess of 20 feet to the ground below.
