The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: In the processing and yard area, located at 8910 NW 13th Street, Gainesville, FL: On or about 10/3/2023, the employer exposed employees to crushed-by and struck-by hazards, in that the Volvo model: L90F, serial: L90FV68380 wheeled loader with fork attachment had inoperable safety devices, to whit: horn, rotating beacon strobe, back-up alarm. The wheeled loader had not been removed from service.
