On or about October 3, 2023, C.A. Jones did not ensure that employees, working at 289 Crestfield Court in Saint Charles, Missouri, were protected by protective helmets, where there was a possible danger of head injury from impact or from falling objects such as cut siding or tools.

29 CFR  1926.451(b)(5)(ii):The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, nor did it have guardrails which blocked employee access to the cantilevered end.  On or about 3 October 2023, two employees of CA Jones were working on a scaffold working on a residential home located at 283 Crestfield Court in Saint Charles, Missouri. The employer did not ensure that the end of a platform greater than 10 feet in length extending over its support more than 18 inches was installed so that the end of the platform was able to support the employee without tipping or had a guardrail that blocked employee access to the cantilevered end.

29 CFR  1926.451(e)(4)(xii):Guardrails meeting the requirements of paragraph (g)(4) of this section were not provided on the open sides and ends of each landing.  On or about 3 October 2023, two employees of CA Jones were working on a scaffold more than 10 feet above a lower level working on a residential home located at 283 Crestfield Court in Saint Charles, Missouri. They were not utilizing guardrail systems at the ends of the scaffold. The employees were observed on the scaffold without guardrails at the ends exposing them to falls to the ground below.

On or about 3 October 2023, two employees of CA Jones were working on a scaffold more than 10 feet above a lower level working on a residential home located at 283 Crestfield Court in Saint Charles, Missouri. They were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the scaffold at a level above 10 feet without fall protection, exposing them to falls to the ground below.

They were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employee was observed on the roof of the building without fall protection, exposing them to falls to the ground below.  REPEAT - C.A. Jones was previously citied for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1616548, citation number 1, item number 2 and was affirmed as a final order on10/12/2022, with respect to a workplace located on Lot #12 Orchard Lakes Circle in Belleville, IL 62226
