29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 29 CFR 1910.147(c)(4)(ii)(B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; 29 CFR 1910.147(c)(4)(ii)(C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and 29 CFR 1910.147(c)(4)(ii)(D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. a) On or about September 29, 2023, the employer did not develop, document, and utilize machine-specific procedures for the drive system on the Hardinge HC Lathe in that the procedures did not include shutdown procedures, procedures for the placement, removal, and transfer of lockout devices, and procedures for testing a machine to verify lockout device effectiveness while unsticking an actuating rod that had become jammed. Employees were exposed to electrical and pneumatic energy.

29 CFR 1910.147(c)(7)(i)(A): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or about September 29, 2023, the employer did not train Harding HC lathe Operators as authorized employees in the recognition of the hazardous energy sources, the type and magnitude of the energy, and methods and means necessary for energy isolation and control, in that Harding HC lathe operators were exposed to ingoing nip points, and pinch points while unjamming the actuating rod in the drive system.

29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (d)(1) Preparation for shutdown. Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazard of the energy to be controlled, and the method or means to control the energy; (d)(2) Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage; (d)(3) Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s); (d)(4) Lockout or tagout device application. (d)(4)(i) Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. (d)(4)(ii) Lockout devices, where used, shall be affixed in a manner to that will hold the energy isolating devices in a "safe" or "off" position. (d)(5)(i) Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. (d)(5)(ii) If there is a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation shall be continued until the servicing or maintenance is completed, or until the possibility of such accumulation no longer exists. (d)(6) Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and de-energization of the machine or equipment have been accomplished. a) On or about September 29, 2023, the employer did not ensure that (d)(1) through (d)(6) were followed by employees performing servicing and maintenance on equipment such as but not limited to; the Hardinge HC lathe. Employees did not properly secure each energy source including electrical energy and pnuematic prior to clearing jams, exposing employees to caught in hazards and ingoing nip points on the belt and pully system.
