29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) On or about October 4, 2023, at the job site located at  8989 North Port Washington Road, Bayside WI,  employees working on a tubular scaffolding were not utilizing fall protection which exposed the employees to an 18 foot 5 inch fall hazard.  (b) On or about October 4, 2023, at the jobsite located at 8989 North Port Washington Road, Bayside WI, employees were conducting grinding work from a scaffold with no top rails in place, and the middle planks were not secured in place where the employees were working. This exposed employees to a fall hazard of seventeen (17) feet or greater to the ground below.
