29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) Roof: On or about 10/2/2023, the employer exposed employees to struck-by hazards while installing plywood sheathing, in that protective eyewear was not provided to employees engaged in framing activities using pneumatic guns at 120 PSI.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Roof: On or about 10/2/2023, the employer exposed employees to fall hazards of approximately 20-feet, 6-inches to the lower ground level, in that the use of personal fall protection was not enforced while installing roof sheathing on the 3/12 pitch roof.
